SECONDARY MANIA DUE TO HYPERTHYROIDISM IN A GENDER IDENTITY DISORDER PATIENT

Abstract

|
Transsexualism is a disorder of gender of gender .identity. An individual, despite having the anatomical characteristics of a given sex, has the self -perception of being of the other sex. This disorder is rare, with estimates of 30,000 cases worldwide. Transsexuals most commonly present by requesting sex reassingrunent change in their physical appearance (usually by hormonal and surgical means) to correspond with their self - perceived gender. There are no well-establishes or exhaustive explanations for the development of transsexualism. Individuals who request sex reassignment require careful and patient evaluation by a psychiatrist with experiencein the management of gender identity disorders. Supportive psychotherapy can serve various purposes in the transsexual individual, but psychoanalysis is generally not indicated due to their poor ego functioning. Sexual reassignment surgery(SRS) to the oppsite gender has been the most widely used and studied treatment modeality for transsexualism. Psychiatric symptomatology in endocrine disorders has long been recognized. Hyperthuroidism (or thyrotoxicosis) is the clinical syndrome resulting from the chronic exposure of the body tissue to excess quantites of thyroid hormone. Mental changes in hyperthytoidism are anxiety with irritability, emotional lability, a feeling of apprehension, in ability to concentrate and cognitive changes including short attention span, distractibility and decreased recent memory. A hyperthyroidism may be mistaken for a -primary depressive disorder, dementia, anorexid nervosa, bipolar disorder, schizophrenia, psycl-iotic depression and organic mental disordes of other etiology.
We experienced a female patient with secondary manic state due to hyperthyroidism, who had been suffered from the transsexual wishes since her chuldhood.
